Our Continuum of Care

Our campus provides a continuum of care in one convenient location. We offer assisted living at Kindred Assisted Living - Fountains and transitional care and rehabilitation at Kindred Transitional Care and Rehabilitation – The Greens. Our campus model allows patients to receive additional health care should their needs change. Residents, patients and families have the assurance of knowing they have access to the care they require without moving campuses.

At Kindred Assisted Living - Fountains, seniors can continue living independently while receiving the extra measure of support they need. Residents have the option of studio, one- and two-bedroom apartments, all with private baths and kitchens. The monthly rental fee includes gourmet meals, housekeeping, transportation, activities and much more. Personal care and medication management are under the supervision of a nurse. Kindred Assisted Living - Fountains residents have priority admission status to Kindred Transitional Care and Rehabilitation – The Greens if they require rehabilitative care.

Kindred Transitional Care and Rehabilitation – The Greens adjoins Kindred Assisted Living - Fountains. Our transitional nursing and rehabilitation center specializes in short-term, inpatient rehabilitation, bridging the gap between hospital and home for patients needing continued care to recover completely. Patients and residents at Kindred Transitional Care and Rehabilitation — The Greens receive preferred admission status to our assisted living residence, provided they meet the criteria.
